This has been the missing link for some time now for folks wanting to convert to a disc brake system on a old/midschool ride and still utilize the larger thread for a freewheel. This adapter fits onto the small side of your flip flop hub. For whatever reason, the only thing you'll find on amazon and the rest of the dinnernet is threaded for the large side. This adapter with the smaller thread is essential if you plan on running a 16T or larger freewheel.
